Section 1: Identification and Application History Section 2: Residency Status Section 3: Equitable Admissions Information Section 4: Pre-requisite course information Section 5: Current Degree, Previous Degree and Course Upgrade Information Section 6: Personal Statement Section 7: Requirement and Expectations of Students admitted to the MSc(OT) program Section 8: Criminal Records Check Please note: - Applicants who already have a Dalhousie Banner number are required to provide it. Applicants who do not already have a Banner number can skip question #4.
- All Sections of this form are mandatory to complete. You will not be able to move to the Next page or Back a page unless you complete each question. You can edit your responses until you submit the application.
- You do NOT need to click save on each page. The save button is an option if you need to stop your applicaiton and return to it at a later time. When you click Save on any page, you will be prompted for information so you can receive a link via email to complete the application at a later date.
