Introduction

Welcome and thank you for participating in this survey. Your opinions are important to us, and we appreciate your feedback. The survey will take less than 10 minutes to complete.

The purpose of this survey is to determine the current capabilities of the information technology used in Urgent Care or Emergency Departments accross Canada; these software are called Emergency Department Information Systems (EDIS).

You have been contacted to provide high-level details about your organization's EDIS system functions. There is currently no inventory of which systems are being used across Canada. Specialized knowledge is not required; anyone who has used EDIS in their work-role will be capable of answering the survey questions.

All participants will have the option to be entered into a lottery and five winners will receive a gift card of 25 $CAD. This study has been reviewed by the Institutional Ethics Review Board of Dalhousie University and it has been determined that it is exempt from REB approval as per TCPS 2.1 policy.

All participants can withdraw from the survey at any time. Participants will not be identified by the data provided.
