Health & Well-being Survey for School-aged Children & Youth During the COVID-19 Pandemic

Information about this survey

You are invited to participate in a survey about activity levels and mood in children and youth. This is a survey for parents or guardians of children in elementary school. The survey is about your child’s activity levels, mood, and overall well-being, and how these may have changed during the COVID-19 pandemic.

We are doing this survey because we want to see what your child’s mood and activity levels are like during the COVID-19 pandemic. It should take about 20-30 minutes to answer the questions.

Gift Draw

By finishing the survey, you will have the chance to enter into a draw for one of ten $25 gift cards. You can choose between a grocery store, SportChek or Chapters gift card if you win. You will need to give us your name and email (to be entered into the draw) but this information will not be linked to your survey answers and will be destroyed after the draw takes place. We anticipate that at least 1000 people will participate in this research study. You should be aware that the odds of winning a gift card are no higher than 1% (less than 1 chance in 100).

Survey Results

If you are interested in finding out the results of this research study, you can tell us and we will send you a summary. You can also tell us then if you are interested in being part of future research studies about your child’s health and well-being. In this case, we will keep your contact information on file, but will only contact you for the purposes of future research.


Your participation in this survey is entirely your choice. Your answers are confidential, which means that they are kept private and are not shared with other people. We do not ask your name.

Despite not asking your name, you do need to know that there is a risk of being identified from the information you provide, like ethnicity, postal code, school, and grade information. If there is a question you do not wish to answer, you can skip it and leave it blank.

There are no right or wrong answers, and if there is a question you do not want to answer, you can leave it blank. You can stop the survey at any time by closing your browser if you no longer want to participate. If you do complete the survey and you change your mind later, we will not be able to remove your answers as we will not know which response is yours.

All of the information you give us will be stored, backed-up and managed only in Canada (at Dalhousie University). There are two versions of the survey, one for parents of younger children (this survey), and one for older children and youth. We will analyze the surveys separately but will look for themes across both surveys. Some of your responses may be included in future presentations and publication of the research results. All information will be saved for up to five years and may be shared for future research, presentations, and publications.
